There are several factors affecting delivery and deposition of AP. The success or failure of optimal delivery of the drug greatly impacts its effectiveness.

I feel optimal delivery of AP to the lungs depends on both the inhalation technique and the pulmonary function of the recipient. These factors will determine the quantity of aerosol deposition and its distribution in the respiratory tract.

The goal of AP is to enhance the deposition of pentamidine to all aspects of the lung fields. Yet some 20-30% of all patients using AP will develop PCP in the upper lobes after receiving AP. I suggest this is due primarily to an improper breathing technique being used.
