The MRC plans to start a trial for 200 individuals at major London and non-London hospitals in the UK in the next few months. This trial will explore whether following maximal suppression of HIV replication, the number of drugs can be reduced without a significant rise in HIV replication. Such an approach would have advantages in terms of convenience, less side-effects and less cost.

All participants in ProCom will receive treatment with four drugs for the first four months - two nucleoside analogue reverse transcriptase inhibitors (ddI plus d4T) plus two protease inhibitors (nelfinavir plus new formulation saquinavir). Treatment will then continue with either all four drugs or with only two drugs (either ddI plus d4T, or nelfinavir plus saquinavir) for another 16 months. If treatment is considered to be failing, either in terms of the viral load, CD4 count or clinical disease, participants may receive the four drugs on an open basis.

People with a CD4 count below 350 or a viral load measurement above 20,000 will be eligible if they are starting HIV treatment for the first time or have had prior treatment (but none for a month before joining the trial) with AZT, 3TC, ddC or loviride.
