Sueanne Lineberger said she spent a night in the emergency room but doctors were unable to tell her what was in the needle and referred her to an outside lab, WGAA-TV, Dallas, reported Thursday.
"You need to be tested for HIV," she said they told her. "We're going to start you on an anti-HIV medicine but you need to be tested for it."
American Airlines' spokesman Tim Smith said the company does not believe the needle was left on the plane intentionally.
"We believe it was an unfortunate accident, which we regret," he said.
Smith said a cleaning crew had entered the plane during a 35-minute layover, but it is possible they missed a needle wedged in the seat.
