The 48-year-old pop star, who is in Malawi for charity, was reported by a Malawian government spokesman to have adopted a one-year-old boy.
"She asked us to identify boys only, which we have done after visiting four orphanages in Lilongwe," government spokesman Adrina Michiela was reported as saying by AP.
Michiela apparently also indicated Madonna had wanted to adopt a girl but changed her mind.
The singer, her film director husband, Guy Ritchie, and their two children flew into the country on Wednesday morning.
Madonna's publicist, Liz Rosenberg, was quick to deny the singer had added to her brood.
"I have no information or confirmation regarding Madonna adopting an individual child. She's kind of adopting an entire country of children," she said in an e-mail to AP in Los Angeles.
"She is on a private visit to Malawi and is involved in the building of an orphanage/child-care centre as well as other initiatives to help the children of that country who have lost parents to Aids."
Madonna's project, called Raising Malawi, aims to set up an orphan care centre to provide food, education and shelter for as many as 4000 children. It will have projects based on Kabbalah, the Jewish mysticism to which the singer is devoted.
