Yvonne Morrison, 24, from Scotland, was arrested in Belfast city centre after she allegedly threw a bottle at an ambulance and injured a paramedic.
Ms Morrison, who was living with a partner at Kilburn Street, Belfast, is accused of assault.
She is also wanted by Scottish police who have seven warrants for her arrest.
The court heard that as Ms Morrison was being arrested earlier this month, she kicked and spat on police, who for their own safety placed her face down on the ground and put a guard over her mouth.
Her defence lawyer said Ms Morrison had been a heroin addict, but had "kicked the habit and had come to Northern Ireland to escape a drug culture where she had been living in Scotland".
He said his client's behaviour had been "caused by a mixture of medication and alcohol".
Mr Justice Weatherup described her behaviour as "atrocious", but said he was granting bail on condition she was immediately re-arrested on foot of the outstanding warrants and taken back to Scotland.
Ms Morrison told the judge: "I hope I don't get killed in a Scottish prison because that's what can happen."
