Peter Ioannou, 39, from Pembrokeshire, denies two counts of rape and one of indecent assault.
The woman, who cannot be identified, said Ioannou threatened to kill her and waved a baseball bat to force her to submit during the alleged attack at a house in Letterston, near Fishguard.
She said she was sure he wanted to infect her, but tests proved negative.
The woman gave evidence from behind a screen at the court, after telling the jury she could not bear to look Ioannou in the face.
She sobbed frequently as she described to the court how the the attack started with an attempt by the defendant to tie her hands together.
She told the jury that he threatened to kill her, partially strangled her and waved a baseball bat at her threateningly.
The woman said she was convinced Ioannou was trying to give her the HIV virus. Later tests showed she was in the clear.
She said she managed to calm him down but a short while late he flew into another rage.
'Apolgising for his conduct'
She said Ioannou twice raped her and at one stage forced her to sniff from a bottle of liquid, which the prosecution claims contained a drug.
She claimed he then told her: "I hope I have given you HIV."
Prosecutor Kevin Riordan said after the alleged attack Ioannou drove to London and that during the journey he made several telephone calls to her.
He said: "The crown will say that this speaks volumes because in effect it amounts to the defendant apologising for his conduct."
Mr Riordan said the defendant told the interviewing officers he had no idea why the woman would make allegations of sexual assault against him.
Cross examined by Geraint Walters, defending, the woman agreed that she had once made complaints against both her grandfathers about incidents that happened to her when she was eight years of age.
