Nosocomial Transmission of Mycobacterium Bovis Resistant to 11 Drugs in People With Advanced HIV-1 Infection

Abstract: Researchers from Hospital Ramon y Cajal in Madrid, Spain, investigated the presence of Mycobacterium bovis in a tuberculosis outbreak at the hospital. In the study, 19 patients diagnosed with multidrug-resistant (MDR) tuberculosis due to M. bovis were compared to a control group of 33 randomly selected patients with HIV-1 and a strain of M. tuberculosis susceptible to either isoniazid, rifampicin, or both. The researchers identified 19 cases in the HIV-1 infected patients of primary MDR TB as a result of M. bovis resistant to 11 anti-TB medications. The index case and two other cases in the case group were found to have had previous contact with another hospital that had had an MDR TB epidemic. The researchers note that despite multidrug therapy with both first-line and second-line anti-TB drugs, all patients died after an average of 44 days. They report that "advanced HIV-1 immunosuppression was associated with the development of MDR tuberculosis," and that the implementation of control measures halted the outbreak.
