Comorbidities of HIV-1/AIDS in Adults

The comorbidities of HIV-1 and AIDS are complex and varied. During the course of HIV-1 infection, an individual is rarely diagnosed with a single health-related problem or disease. It is, therefore, necessary for nurses and physicians to understand the spectrum of HIV-1 disease and the individual biobehavioral and psychobehavioral responses of patients infected with HIV-1. For example, intravenous drug users have numerous preexisting health problems. The most notable diseases affecting them involve pyogenic bacterial infections, commonly seen in pneumonia, endocarditis with sepsis, and tuberculosis. In the United States, issues such as cost control, prospective payment, and managed care have led to a mass movement to develop critical clinical pathways to plan and measure outcomes of care. Such strategies often fail to recognize the complexities of HIV/AIDS patients for whom-- because of their disease and inherent comorbidities--it would be extremely difficult to develop clinical pathways of care.
