Abstract: Barnes et al. (New England Journal of Medicine--June 6) failed to recognize in their article on tuberculosis patients with HIV the manner in which HIV infection accelerates the development of tuberculosis when tuberculin-negative HIV-positive indificuals are infected by Mycobacterium tuberculosis. William Stead and John Lofgren of the Arkansas Department of Health write that recent reports indicate that active tuberculosis may significantly increase the progression of HIV infection. Hospitals and clinics providing care for persons with HIV should therefore use upper-air sterilization by the germicidal portion of the ultraviolet spectrum. If air recirculation continues to occur air should be irradiated with germicidal ultraviolet light or filtered through a high efficiency particulate air filter. This would exhaust all resources to prevent the transmission of tubercle bacilli to particularly susceptible individuals. Taking action would be especially important as the spread of organisms resistant to multiple drugs become more prevalent, the researchers conclude.
