"Extrapulmonary Pneumocystosis in Patients Taking Aerosolised Pentamidine"

Abstract: Because pentamidine received via aerosol does not result in systemic distribution, it cannot be expected to suppress extrapulmonary Pneumocystis carinii pneumonia (PCP) infection, writes Donald W. Northfelt of San Francisco General Hospital. A review of patients who received prophylactic aerosolized pentamidine and developed extrapulmonary PCP infection, writes Northfelt, reveals several common features: widely disseminated infection, often without concurrent pneumonia; long duration of AIDS; prolonged use of aerosolized pentamidine; and concurrent treatment with zidovudine (AZT). Aerosolized pentamidine and AZT in prolonged use, writes Northfelt, may allow for the emergence of more insidious subacute disorders such as extrapulmonary PCP. Prolonged surveillance, the researcher writes, is necessary to determine the incidence of extrapulmonary PCP and the need for systemic prophylaxis to prevent disseminated PCP in HIV-infected patients.
