"Nosocomial Epidemic of Active Tuberculosis Among HIV-Infected Patients"

Abstract: Giovanni Di Perri and colleagues of Verona, Italy report on a study of 18 HIV-infected patients who were exposed to Mycobacterium tuberculosis in hospitals. Eight of the patients developed active tuberculosis. Active tuberculosis infection correlated with lower total lymphocyte and CD4 lymphocyte count, the researchers report, but not with previous infection by M. tuberculosis. Previous reports have found that development of tuberculosis corresponds to previous positive tuberculin skin tests in IV drug users, but in this study, Di Perri and colleagues say, a skin reaction corresponded to a still-functioning immune system in patients who did not develop active tuberculosis. Four of 18 tested positive for previous exposure to M. tuberculosis and did not develop active infection. The physicians conclude that they have found an outbreak of active tuberculin infection, rather than a reactivation of latent infection.
