Preliminary Study of a 'Values History' Advance Directive Interview in a Pediatric HIV Clinic

To facilitate advance-care planning discussions among families and careproviders at our pediatric HIV clinic, we developed a nine-item interview guide, using techniques of medical family therapy. We tested this interview guide with 10 families at the clinic who agreed to participate in the study, including two families with teens who knew their diagnosis. The interview guide elicited many of the elements that should appear in a values history, and can provide a structure for families as they began discussions regarding decision making and end-of-life care. Further study is needed to determine how many families would agree to participate in such discussions, the appropriate age range for child participants, and the impact of these discussions on medical care and outcomes.
