Some Ethical Issues Associated with HIV Vaccine Trials

I shall not review the underlying ethical principles of respect for persons, beneficence, and justice from which we derive our procedures, as these have been discussed in detail elsewhere. The first expectation of any project is good research design. That includes such questions as these: Is the proposed vaccine promising? On the basis of what data? Lacking a satisfactory animal model, what preliminary data should we expect before turning to human subjects? Preliminary vaccine data will remain, at least for the time being, unsatisfactory compared with other types of clinical trials, but it would be unwise to await a greater understanding of AIDS or a scientific breakthrough before proceeding with vaccine trials.
