Vigorous debate has been focused for the past year and a half on public health programs designed to prevent the spread of HIV. These programs, of course, merit very high priority. We can now see--unfortunately, clearly--that rational programs will be implemented at a glacial pace and with maximum reluctance, while emotional and meaningless (but very costly) programs such as premarital testing will be lionized. In any event, most of the people who will require health care through 1992, and possibly through 1995, are already infected. I would like to turn to what we will have to do for them in terms of planning access to health care.
