Case Management and Support Services

1. Start by choosing a case manager from the list below. Case managers can help you and your family make a plan to get the medical services and community support services you may need.
   
2. Call your case manager to see if you meet the requirements and to be signed up.
3. If you do not have a case manager, look at the list below. Your case manager will help you fill out the forms and send them to the Treatment and Care Program. Some services offered under the Supportive Services Program require a referral; your case manager will fill that out and send it in to the Treatment and Care Program.

Waiting List? - Sometimes a state has a waiting list people have to stay on until spaces become available. Check to see if there is a waiting list for the program by contacting them directly. According to NASTAD's most recent ADAP Watch Utah does not have a waiting list.

You may need to try a Patient Assistance Program or apply for Medicaid or Medicare or private insurance. Do not ever drop your private insurance without first seeing if your state will cover part or the whole premium, especially if you have children. Please do not send completed Patient Assistance Program applications to The Access Project.

Case Management and Support Services : Find case management services, health care provider, or a network of service providers and community based organizations in a state

HIV Treatments : Listings of Treatments for people with AIDS and HIV related conditions, including preventive therapies and links to more information

Hepatitis Treatments and Vaccines : Listings of hepatitis vaccinations, immunizations, and treatments for Hepatitis C and Hepatitis B. This includes currently approved treatments and drugs in development.

Patient Assistance and Expanded Access Programs : Drugs available through Expanded Access and Patient Assistance Programs, including HIV and Hepatitis treatments and vaccines.

Educational and Informational Resources : A library of fact sheets, reports, training resources and internet links to inform, educate, and encourage collaboration and networking among communities and providers.

Simple Facts Sheets : Fact sheets on HIV and Hepatitis treatments, conditions, symptoms, research studies, and other topics. The purpose of The Simple Facts Sheets are to take complicated subjects and make them accessible.

Health and Living : Information and Links to Facts, Tips and Advice on living your life, including enjoying the present and planning for the future. Includes Food, Entertainment, Relationships, Disease Prevention, and other topics.
