Financial Eligibility
- New York State Residency (U.S. Citizenship is not required)
- Income < $44,000 for a household of one, < $59,200 for two,
< $74,400 for three or more
- Liquid assets (Cash, savings, stocks, bonds, etc.) < $25,000
Medical Eligibility
- ADAP & ADAP Plus: HIV+
- Home Care: AIDS or HIV illness and chronic medical dependency due to physical or cognitive impairment from HIV infection
Enrollment - A single application form can be used to enroll in ADAP, ADAP Plus and the HIV Home Care Program.
- Proof of residency, income and assets
- A Medical Application signed by a licensed medical professional is required, verifying HIV-infection and indicating the applicant's medical status
- A treatment plan by a physician must also be submitted for Home Care applicants
Enrollment in the programs is coordinated for all eligible individuals. Upon determination of eligibility, an Enrollment Card is issued which may be used to receive care from an enrolled provider. The programs provide an automated eligibility verification system to allow providers to confirm participant eligibility status.
ADAP/ADAP Plus has enrolled over 2,970 local pharmacies, 354 hospitals, clinics and drug treatment programs, 645 private physicians, and 101 laboratories to participate. The HIV Home Care Program has enrolled 247 agencies.
Other Programs
- ADAP Plus provides primary care and other medical services including viral load testing. Home Care Program covers skilled nursing, home health aide services, IV therapy administration, lab tests, ADAP medications & supplies. There is a limit of $30,000 on home care services
- APIC - Insurance Continuation: Policy must be cost effective. Insurance coverage must be comprehensive. If the policy is work-related, premiums must be greater than 4% of income and the employer share must be greater than 50% of premium cost
- Nutritional - Nutritional assessment and counseling available.
For people with symptomatic HIV disease or AIDS, 12 visits per year are allowed. For asymptomatic people with HIV, 4 visits a year are allowed.
Maximum oral nutritional supplement allowance is 24 oz./day.
Supplements available: Multiple Vitamins and Minerals, Beta Carotene, Vitamin B6 (Pyridoxine), Vitamin B12 (sublingual), Vitamin B12 (intramuscular), Vitamin C (ascorbic acid), Folate, Folinic Acid (leucovorin), Iron, Lactaid, Magnesium Glutamate, Selenium.
Oral nutritional supplements which are included in the Medicaid Formulary (including pediatric) are covered. Supplements, vitamins, and minerals will be covered only with a prescription and when dispensed at an ADAP enrolled pharmacy.
