CARE Programs

The New Hampshire CARE Program offers the following services:

  • medication coverage (ADAP)
  • health insurance continuation
  • primary and specialty care services
  • home health care
  • case management
To be eligible for these programs there are two options:

Option 1: To be eligible for medical assistance through the Ryan White CARE Program:

  • income less than 300% of federal poverty level
  • CD4 below 350
  • No Asset Requirements
  • Must Be NH Resident
  • Must apply for Medicaid and receive a denial letter (reapply every year for Medicaid)
  • Re-enrollment every 6 months

Option 2: Same as above, but when your income exceeds 300% of the FPL you are eligible for medical assistance after you expend a specific amount of your own income annually. In this case, 80% of the difference between your annual income and the maximum annual income for your household must be spent before you are eligible for DHHS assistance with your medical expenses.


ADAP Eligibility

New Hampshire ADAP is one of the CARE programs described above. Open enrollment. There is no cap on the program. The NH CARE Program formulary covers all FDA approved antiretroviral and opportunistic infection-related medications. Some require prior approval. If you do not have a copy of the NH Medicaid PDL, please call the CARE program at 603-271-4480 for a faxed copy or you can download one

Drugs are dispenced at the client's preferred pharmacy (1 only) and the pharmacy signs a contract with the ADAP Program (fee for service agreement).

Patient Assistance Programs

Sometimes you must look for other programs to meet your needs. Don't drop your insurance coverage or COBRA premiums without first seeing if your state will cover part or the whole premium or drug cost co-pays. Apply to get Medicaid or Medicare or private insurance with the help of your case manager. You might also need to try a Patient Assistance Program. The pharmaceutical industry has set up programs that offer medications for free, or sometimes at a very low price. You may have to apply directly to a specific company. Your doctor or another member of your health care team like a case manager will usually help you out to apply, and you may need to apply to more than one company for the treatments you need. If you have HIV and/or chronic hepatitis, we can usually help you.
