Financial Eligibility
  • Income at or below 300% of the current Federal Poverty Level.
  • Liquid assets less than $25,000 (not including home and car).
Medical Eligibility
  • HIV +
  • Prescription for a covered drug.

  • Must be a A resident of Minnesota or St. Croix or Pierce counties in Wisconsin.
  • Must be currently uninsured or insured with ≥ 20% of drug cost or drug copay.
  • Must not be on Medical Assistance, General Assistance Medical Care or MinnesotaCare .
Pharmacy Drugs may be purchased at any outpatient pharmacy that is a Minnesota Health Care Programs provider.
Nutrition Payment of up to $50 each month for enteral nutrition supplements, when prescribed by a physician. Generic vitamines are covered.
Other Services Payment of medical and dental insurance premiums, including, but not limited to: policies carried through a COBRA extension, Medicare Supplement Policies and conversion or individually purchased policies. An employer must be paying 50 percent or less of an insurance premium for clients to be reimbursed for their portion.
