Are you eligible for services?

Contacting your local County Health Department is a good way to apply for any of the three programs described below. You can call 1 (800)352-2437 as well. They have brochures and application forms as well.

What services are covered?

  • Doctor visits, Case Management, Dental, Prescriptions, Transportation, Housing, Health Insurance Premiums, Other Support Services

These services are covered through these three programs :

  • AIDS Drug Assistance Program (ADAP)
  • AIDS Insurance Continuation Program (AICP)
  • State Housing Opportunities for Persons with AIDS (HOPWA)

To be eligible for any of these programs you must meet the following criteria:

  • You have to be HIV positive
  • You must be living in Florida
  • You cannot be receiving the same services from Medicaid, Project AIDS Care or Insurance
  • You have to be low-income - at or below 300% of the Federal Poverty Level (FPL), and $12,000 or less in cash assets

There are Program Qualifications for enrollment in one of the following single service programs. If you do not meet these Program Qualifications, your eligibility status still allows you to receive these services from other HIV/AIDS Patient Care Programs, if funded and available.

  • ADAP requires a prescription on the drug formulary
  • AICP requires a health insurance policy
  • HOPWA requires 80% of the median area income which may be more restrictive than the 300%

What documentation is required?

The information you give about your income and family situation may be checked, including computer matches. You must agree to let the Department of Health's eligibility staff obtain needed information to make a determination of eligibility. The Department of Health, eligibility staff cannot discriminate because of race, color, sex, age, disability, religion, nationality or political belief. You can ask for a fair hearing if you think the decision made on your case is unfair or incorrect.

There are several types of documentation you must provide:

  • Part 1: Proof of HIV Status - Original or copy of an HIV Laboratory Test which shows your HIV Status.Do Not Bring a Doctor's Statement

  • Part 2: Living in Florida. Bring Photo ID if possible

  • Part 3: Bring all documents that show you are receiving services from other programs or have been denied, such as:
    • Medicaid Card
    • Medicare Card
    • Project AIDS Care Eligibility Notice
    • Insurance Policy
    • Public Assistance Program Award or Denial Letters
    • Veterans Administration and Other Documents which apply.

  • Parts 4 and 5: Bring all that apply to show your Income, Cash and Items of Value, such as :
    • Current Pay Stubs (last 3 months if possible)
    • Bank & Savings Statements
    • Military and Earnings Statement
    • Rental and Other Income
    • If Self Employed - 1040 Form
    • Public Assistance Award or Denial Letters

If you don't have all the documentation, include what you have or call the local office for help. Not having the right documents could slow the process down.

When will I start receiving services?

When your application and the documentation is received you will be scheduled an eligibility interview with eligibility staff to review the application. You may be asked to submit more information.

Eligibility staff will give you an eligibility confirmation letter after your application is processed and approved. You will be asked to choose a case man- agement agency to help you determine all of your needs and/or you may be referred to one of the single service programs for drugs, housing or insurance assis- tance depending on your situation.


What if I am not eligible and I don't like the decision? - You will receive a written letter explaining why you are not eligible, and you will be provided information about your right to ask for a fair hearing. People often get turned down the first time they apply for asssistance programs, usually because they have not filled in forms correctly, or they have not provided the right documents. Many people work with a state or community-based case management agency to assist them with the process before they apply, and if they are turned down.

fiber optics phooPatient Assistance Programs : Sometimes a state has a waiting list until certain programs become available. You may need to try a Patient Assistance Program, or try to get Medicaid or Medicare or private insurance first. Do not ever drop your private insurance without first seeing if your state will cover part or the whole premium, especially if you have children. The pharmaceutical industry has set up programs that offer medications for free, or sometimes at a very low price. You may have to apply directly to a specific company, or use a national service set up by a group of companies like pparx.org. Your doctor or another member of your health care team like a case manager will usually help you out to apply, and you may need to apply to more than one company for the treatments you need.
