Program Eligibility
The patient must meet the following criteria to qualify:
  • Has no health insurance or no prescription coverage.
  • Not eligible for Medicaid, ADAP, Medicare Part D, or other drug assistance programs.
  • Financial eligibility is evaluated on a case-by-case basis.
Enrollment Process
Doctors and patients can call to enroll. An application is sent to the doctor's office for completion by both.
Re-Application
Once accepted into the program, a re-evaluation form will be sent to the doctor's office. This will have to be completed and sent back to the program in order to continue receiving drug.
