Program Eligibility
The patient must meet the following criteria to qualify:
  • Has no health insurance or no prescription coverage.
  • Income information must be provided, including assets such as stocks and bonds.
Enrollment Process
To receive an application the doctor or patient must call. An application form will be faxed. Application forms can also be obtained from their website at www.procritline.com
Re-Application
Application must be renewed every 6-months.
Comments
The approval process takes 24-48-hours after the receipt of the enrollee's information. Within 3-5 business days a card will arrive in the mail. On the card will be a number that is good for 1-months supply of the drug from a certified pharmacy. Once this number is used the drugs for the remaining 5-months will become available.
