Program Eligibility
The patient must meet the following criteria to qualify:
  • Has no health insurance or no prescription coverage.
  • Not eligible for Medicaid, ADAP, or other drug assistance programs.
  • Financial eligibility evaluated on a case-by-case basis.
Enrollment Process
Doctors or patients can call to enroll. Basic pre-screening questions are asked during the initial call. Once these questions are answered an application is sent to the doctor's office for completion.
Re-Application
Doctors must reapply every 3 months.
Comments
After the completed application has been received along with the original prescription, it can take several days for processing to be completed. The drug is delivered via 2-day Fed Ex.
