Program Eligibility
Health care provider must contact the program directly. To expedite the process, the patient's income, insurance status, and medical information should be on hand.
Enrollment Process
Enrollment is done entirely over the phone by the health care provider. After the enrollment has been completed and approved, a card is mailed to the doctor. The patient can use the card for a 30-day supply of the drug. Once the number on the card has been activated a 5-month supply of the drug is sent to the doctor's office.
Re-Application
An application is sent to the doctor's office. Another 6-month supply is sent to the doctor's office when the application is returned. Each application process is good for 12 months.
Comments
It will take 7-10 days for the drug to arrive. In the case of an emergency, special arrangements can be made over the phone for immediate access to a 30-day supply of the prescription.
