Program Eligibility
The patient must meet the following criteria to qualify:
  • Must be a U.S. citizen.
  • Has no health insurance or no prescription coverage.
  • Not eligible for Medicaid, ADAP, or other drug assistance programs.
  • According to the program: "Income criteria is very generous".
Enrollment Process
Patients or doctors can call to get an application. The completed application needs to be faxed back to the program to start enrollment.
Re-Application
After acceptance into the program re-enrollment is not necessary for 12-months.
Comments
It will take approximately 2-days to have the application form completely processed. After the 2-day turnaround the drug will be sent out overnight.
