ISSUES: How do we make 3 by 5a reality in our communities?Stigma and discrimination is still a major challenge to access to treatment most of our brothers and sisters die before they access it. Objectives: To promote comprehensive HIV care and support with emphasis on the PLWAs who are already open and sure of their sero positive status. To improve access to HIV /AIDS related medicines. Project: This was initiated by Sr. Freda Community health nursing home together with CBO BANANDA in Kiungani, a rural area in Western Kenya. This is an innovative project designed to integrate clinic and community based approaches to AIDS patient care. It consists of providing clinical care and counseling services to PLWHAs in Kiungani, Kenya in order to improve their quality of life. The assessment was carried out using variety qualitative research methods such as semi-structured, interviews, focus-group discussions; personal interviews.40 persons living with HIV were voluntarily involved.50 persons, members from the community were also interviewed in order to get their opinion on access to treatment not necessarily that they know their sero status. Discussions: It was observed that PLWHAs as well as the community members had different indices for satisfaction and benefit especially medical care. Aspects of care services was regarded by PLWHAs as being essential as food and drugs were addressed by this project as well as suggestions to improve and sustain current community- based home care services were obtained from perspective of PLWHAs, care givers and community.

LESSONS LEARNT: The result will help to re-design on-going HIV/AIDS patient medical care as well as home based care addressing the priorities of PLWHAs and shift emphasis from the national based approach through government hospitals.
