Incidence of Intestinal parasitic infections in HIV infected and non infected cases.

BACKGROUND: The spore forming protozoa infect both immune competent and immuno deficient hosts.

METHODS: in this case-control study we evaluated stool sample of 75 HIV infected patients and 140 cases of general population(randomly)of kermanshah.

RESULTS: 75HIV infected patients(69male & 6female)and 140 HIV negative cases.(69male & 71 female)were evaluated with age mean of 35.66 years.(+,- 10.89) out of 75 HIV infected patients 35 cases(46.7%)had Intestinal Parasitic Infections(IPI),compare to 20 cases(14.3%)of non HIV cases. According to kind of parasitic in HIV infected patients,20 cases(26.7%) had Cryptosporidium,13 case(17.4%)Entamoba.coli 6cases(8%)Blastosis Hominis,2cases(2.7%)Endolimax-n,two case Isospora(2.7%),one case Giardia L and two case Histolytica(2.7% )one case Ascaris.7 cases(29%) had more than one parasite which three cases had more than two parasits. Based on CD4 count,out of 11,HIV infected cases with CD4 count less than 200,9 cases(81.8%)had intestinal parasitic infections(7casesCryp- p)out of 19 cases with 200<CD4<350,9 patients(47.3%) had IPI.(5 cases Cryp-p,2 cases B. H),out of 19 cases with 350<CD4<500,9 patients(47%)had PIP,(6 cases Cryp-p and 4 cases E.coli coinfected)and out of 26 cases with CD4>500,8 patients(38%)one case Cryp-p and 5 cases E. Coli). In non HIV infected cases,9 cases(6.4%) had blast,6 cases(4.3%)E. coli,2cases(1.4% )Endo-n,1 cases Cryp - p and one case Giardia. There wasn't any coinfection in this group. Discussions: 46.7% of HIV infected cases IPI compare to 14.3% in general population,So the rate of IPI in HIV infected cases is triple. HIV infection increases the rate of IPI coinfection,compare to general population. The rate of Opportunistic Infections(OIs) such as cryp - p in cases (OR=5.2) in CD4 count less than 200.we counclude that detection of IOIs in HIV infected cases,especially in AIDS phase is very important.
