BACKGROUND: Tumour necrosis factor- alpha (TNF-ialpha) levels have known to be elevated in HIV infection as also in dilated cardiomyopathy. HIV infection is also associated with abnormal cardiac functional status. We conducted this study to find if TNF-ialpha levels correlate with cardiac function in HIV infected individuals. Materials and
METHODS: We prospectively studied 17 HIV positive individuals (3 females, 14 males) presenting at outpatient department of All India Institute of Medical Sciences, New Delhi, India. The cardiac functions were evaluated using Echocardiography using Hewlett Packard 5500 sonus using 3.5-5.5 Mhz transducer. Immune status was evaluated using CD4+ lymphocyte count and TNF-ialpha was measured using ELISA. TNF-ialpha levels of 25 volunteers (15 females, 10 males) were also measured as a control. All the values are represented as mean ± S. D.
RESULTS: Mean age of cases was 29.9±7.5 years versus 25.8±6.0 of controls. The TNF-ialpha levels of cases was 26.5±29.8 vs. 5.8±3.4 of controls (p value<0.01). Average CD4 count was 343.6±197.9/ cu. mm. Mean EF% of cases was 56.3±9.5. Average BMI of cases was 20.6±3.3 kg/sq. m. The TNF-ialpha levels did not correlate significantly with cardiac functions(r= -0.286).
CONCLUSIONS: TNF-ialpha levels are elevated in patients with HIV infection but do not correlate well with cardiac function. So, TNF-ialpha levels could not be the cause for cardiac dysfunction in HIV infection however, may be a contributing factor.
