BACKGROUND: Since 1986, the AIDS National Reference Laboratory from Cuba has been using the diagnostic kits for the detection of antibodies to HIV manufactured by Organon Teknika in their different generations, besides the Western blot asssays and others of the algorithm of work., as part of the strategy followed in the process of confirmation of this entity. The new 4th generation kit was tested for its possible inclusion in our present scheme.
METHODS: A total of 1337 sera samples, taken during the year 2000, from different population groups, were studied. They were tested using the kits Vironostika HIV Uni-Form II plus 0 (3rd generation) and Vironostika HIV Uni-Form II Ag/Ab (4th generation). Boxtel, the Netherlands. The confirmation of reactive samples was performed by Western blot (DAVIH Laboratories, Cuba)
RESULTS: Of all the tested samples (9.5%) were anti HIV-1 reactive and 32 (2,4%) indeterminate. When using both kits, 22 samples (1,6%) showed discordant results (reactive in only one assay). The Kappa value was excellent: 0.98. The 127 reactive samples by DAVIH-BLOT showed reactivity in both ELISA systems, yielding a sensitivity of 100%, and a specificity of 99.7% for each.
CONCLUSIONS: The 4th generation assay tested keeps the parameters of sensitivity and specificity required to be included in our working scheme, where all the reactive samples are confirmed by Western blot, for our work as a Reference Laboratory in the diagnosis of HIV/AIDS infection.
