BACKGROUND: The objective is to find out the pregnancy outcome among HIV-1 infected antenatals through prospective study. It is hypothesised that the maternal morbidity and fetal outcome would be adverse among the seropositives.
METHODS: 80 HIV Type-1 seropositive antenatal women and an equal number of controls attending Maternity Hospital, Tirupati, India constituted the study sample. Test for HIV antibodies was conducted using LABSYSTEM's 3rd Generation Peroxidase Kit (HELSINKI,Finland) using indiret solid phase enzyme immunoassay with horseradish peroxidase as marker. 16% opted for therapeutic abortion and the rest were followed throughout pregnancy and 6 weeks postpartum.
RESULTS: 25% positives had no risky sex behavior. 34% seropositives and 11% negatives had sexually transmitted diseases .26% of positives were teens, 38% primis, and 12% had fetal wastage. The figures were less for the controls(22% 12%and 6% respectively). Intra Uterine Growth Retardartio, Premature rupture of membranes were higher among the positives and statistically significa (P value <0.05). The data on preterm labour (24%),Intrauterine death(10%) and puerperal sepsis(8%) was also higher than the controls(10 %,3% and 3% respectively). Neonatal outcome was adverse among the seropositives as evidenced by increased neonatal deaths (6%), stillbirths (9%) and low birth weight (20%) as against 2%, 4% and 10% respectively for the controls.
CONCLUSION: HIV infection is an important determinant of maternal morbidity and perinatal mortality. It is imperative for the Government of India to provide 'antiretroviral therapy' free of cost to those in need.
