ISSUES: The State of Jalisco, with 124 districts, in 1996 had six millions of inhabitants and 3,586 accumulate AIDS cases, even so there was only one State HIV/AIDS Information Center in the capital city, Guadalajara, while the new HIV infections, were increasing in rural zones, in association with heterosexual transmission and migrant population.
DESCRIPTION: The AIDS State Council in Jalisco (COESIDA) began in 1997 the Decentralization Program, according with the priority strategies of UNAIDS: Join efforts for the optimization of the resources at all levels; encourage efforts by mitigating the stigma; and increase the prevention/control of STDs. The main goal of this program was to expand the scope of the State Information Center to 35 districts. As inclusion criterion were those districts which had the higher AIDS rate in the last five years, as well as, a temporary migrant, rural and poor populations; therefore, in a period of three years we must reach the following objectives: -to develop a new concept which integrates federal, state and local resources to increase the coverage and effectiveness; -to enable the local community to conduct their own HIV/AIDS/STD programs, through the trainning of health personnel, community leaders and health promoters; -to regulate the establishment of HIV/AIDS District Committees (COMUSIDAS) to plan, implement and evaluate by themselves prevention and control programs.
CONCLUSIONS: We developed a valid methodology, that we will share with other countries, with similar policy, economical and social characteristics for its further implementation. It is possible the optimization and integration of human, material and financial resources of the federal, state and local levels in HIV/AIDS/STD programs. The evaluations after the implementation of the local programs have shown that there are an increase of the knowledge level and a better social perception of AIDS among the communities
