Stoppage of antiretroviral treatment during the first trimester of pregnancy: current controversies.

BACKGROUND: The Authors consider the diversification of pharmacological management in pregnancies of HIV seropositive women since HAART was introduced. Is it better to stop or continue antiretroviral therapy during the first trimester of pregnancy? Materials and

METHODS: The Authors studied 26 pregnancies in HIV seropositive women and observe the viral load levels after stoppage of antiretroviral therapy. They compare it with viral load levels when the pharmacological regime was continued.

RESULTS: In the two groups studied viral load levels show clear differences. When we stopped pharmacological treatment, we observed a rebound of viral load levels. When we continued therapy, started before the begin of pregnancy, we observed lower viral load levels. These lower levels of viral load show to be fixed up to the end of the pregnancy.

CONCLUSION: When we stopped therapy during the first trimester of pregnancy, we found some difficolties to obtain a decreasing of viral load levels before the end of pregnancy. Nevertheless it is very difficult the choice to continue pharmacological treatment during the first trimester when we can disturb the organogenesis. The results coming out our study suggest caution when we choose to stop HAART during the first trimester of pregnancy
