One band criteria in urine Western Blot interpretation for HIV-1 positivity.

OBJECTIVE: To determine if reactivity against HIV-1 viral gene product gp 160 only can be the interpretation criteria for positivity in a urine HIV-1 Western Blot.

INTRODUCTION: HIV-1 urine antibodies can be reliably detected in individuals exposed to HIV-1 by the Calypte HIV-1 Urine EIA as demonstrated in a prospective clinical study of 11,147 subjects. Comparing urine and matched serum EIA results for 7,082 low risk subjects, there was 99.1% agreement between urine and serum (7,014/7,075). Urine repeat reactivity (RR) was demonstrated in 296 of 298 AIDS subjects for a 99.3% agreement with serum EIA results. Calypte developed an HIV-1 Western Blot modified for urine in order to confirm EIA results.

METHODS: Confirmation of urine EIA RR results for 825 subjects was carried out with a procedure modification for urine using the Bio-Rad Novapath HIV-1 Immunoblot. Bio-Rad HIV-1 Western Blot interpretation for matched sera was according to the manufacturer's package insert and is a two band criteria for HIV-1 positivity. Band reactivity patterns were compared for the 825 matched urine and serum EIA RR specimens.

RESULTS: For the urine Western Blot results, the gp 160 only band was associated in 824 out of 825 subjects with serum positive results for a sensitivity of 99.9%. Using a two band criteria for urine reduces the sensitivity to 95.6% (789/825). In evaluating the distribution of band reactivities for serum positives gp 160 only was present in all 825 serum Western Blots. In the one instance where a urine did not show a gp 160 band matched to a positive serum, no viral bands were present from the urine sample.

CONCLUSION: A one band criteria, gp 160 reactivity, supports the interpretation for an HIV-1 positive urine Western Blot.
