The impact of AIDS on health care utilization and expenditure by the fatally ill in northwest Tanzania.

In the first wave of a longitudinal survey of households in northwest Tanzania, household members reported on the health care utilization and expenditure by recently deceased household members and relatives. Of the 258 household members who died in the last 12 months, 200 (77%) were reported to have visited a (traditional or modern) health facility at least once, but only 43 (17%) to have visited three times. However, the proportion visiting three times was much higher for AIDS deaths (15% visited the hospital three times) than for non-AIDS deaths (only 3% visited the hospital three times). Households were more likely to report expenditure on medical care prior to death for household members who were close relatives of the household head (77%) than for those who were not (49%). Similarly reported expenditures were larger for close relatives (12,500 1991 Tanzanian shillings) than for those who were not (9,500 Tanzanian shillings). The paper also presents the results of multivariate analysis of reported utilization and expenditure.
