Serum antibody responses following administration of a fourth dose of HIV-1 recombinant gp-160 vaccine to normal volunteers.

OBJECTIVE: To assess the immunogenicity of a fourth dose of an HIV envelope glycoprotein (rgp 160) vaccine in normal volunteers.

METHODS: Either 40ug or 80ug of a recombinant HIV-1 glycoprotein (rgp160) prepared in a baculovirus vector (MicroGene Sys), was administered as a fourth dose of vaccine to 24 normal subjects, who had participated in an earlier 3-dose study of rgp160. The fourth dose was given approximately 12 months after the third dose. Subjects were followed for clinical and laboratory toxicities, and serum antibody responses were determined by ELISA, Western blot, and neutralization assays.

RESULTS: The fourth dose of rgp160 was well tolerated without clinical or laboratory toxicity. Compared to values after the third dose, ELISA values were higher in 6/11 subjects who received a fourth dose of 40ug of rgp160 (p=0.12) and in 8/10 subjects who received a fourth dose of 80ug of rgp160 (p less than 0.05). Western blot reactivity to gp160 and gp41 have been noted in 9/10 subjects to date following the fourth dose. In addition, serum neutralizing antibody activity, which was previously absent in these subjects, has now been detected in 4 of 14 subjects after the fourth dose. (D. Montifiori, submitted).

CONCLUSIONS: Administration of a fourth dose of rgp160 in the above immunization schedule results in augmented serum antibody responses, including the development of neutralizing antibody activity. Further studies of rgp160 as an HIV candidate vaccine are warranted.
