OBJECTIVE: To evaluate prospectively efficacy and safety of single daily dose Fluconazole (F) in the treatment of fungal infections in ARC and AIDS.
METHOD: 1) F, 400 mg on day 1 and 200 mg thereafter p.o. or i.v., was given in an open controlled trial to: a) 1 bronchial and 14 esophageal candidiasis (BC and EC); b) 3 disseminated Cryptococcosis (DC) as first-line therapy and 2 cryptococcal meningitis (CM) as maintenance therapy after a 30 day course of Amphotericin B+ Flucytosine. 2) Treatment of oral candidiasis (OC) was evaluated in 16 patients (pts), 8 ARC and 8 AIDS, randomly assigned to F, 200 mg on day 1 and 100 mg thereafter p.o., or to Ketoconazole (Kc), 200 mg bid p.o., 8 ARC and 10 AIDS. Controls were performed before, during and after therapy.
RESULTS: Cure rates were: 1) 1/1 for BC, 11/13 (84.6%) for EC, with a mean dose of 235 mg F given for a mean of 31 days, with 2 drop-outs; 2) 1/3 DC cured with a 42 day F course, 1 dead on day 6 and 1 discontinued on day 10 because of inefficacy; 3) no recurrences of CM in 2/2 after 6 months maintenance therapy on F; 4) 81.2% cure of OC with F at a mean dose of 108 mg x 15.9 days versus 77.8% with Kc at a mean dose of 375 mg x 15.9 days. F was discontinued in 2 pts because of cutaneous rash (5.5%). An overall 19.4% of F-treated pts had moderate elevation of liver function tests, mostly in individuals with preexistent liver disease and no differences were noted between F versus Kc.
CONCLUSION: F is effective and safe in the treatment of AIDS' fungal infections.
