HIV DRUG RESISTANCE — AN OVERVIEW

The complex connection from development of drug resistance to loss of antiretroviral drug activity to diminished control of virus replication to increased rate of destruction of CD4 lymphocytes to increased risk of opportunistic events to death has confounded simple correlations of antiviral drug resistance to survival. Evidence supporting this relationship has been generated for AZT monotherapy with ACTG trial 116B/117 and more recently with trials of combination nucleoside regimens. The correlation of loss of antiviral drug activity with the emergence of drug resistance has been quite straightforward for the non-nucleoside reverse transcriptase inhibitors and the potent protease inhibitors. Prevention of resistance will prove central to the new chemotherapeutic paradigm of complete suppression of HIV replication with potent combination regimens. The impact upon survival of this strategy is highly likely but not yet confirmed.
