INITIATION OF ARV TREATMENT IN THE REPUBLIC OF ARMENIA

INTRODUCTION: From 1988 to 1 February 2005 305 HIV cases were registered in the Republic of Armenia. Until 2005 ARV treatment has been inaccessible for PLWHA in the country. The National Programme on HIV/AIDS Prevention (supported by the GFATM) envisages treatment for 20 PLWHA. In 2004 the National HIV/AIDS Treatment and Care Protocols were developed and ratified on the basis of HIV/AIDS Treatment and Care Protocols for CIS countries developed by WHO. The National Protocols recommend that PLWHA should start ARV treatment when clinical AIDS stage is present, CD4+ <200/µl or CD4+ <350/µl in symptomatic patients. In order to identify HIV patients having indications for starting ARV treatment 74 PLWHA were tested and examined.

METHODS: The study was conducted in the National Center for AIDS Prevention. Full scale clinical, instrumental examinations and laboratory tests (CD4+ T-lymphocyte, Hepatitis-B and C) are performed during initial evaluation of PLWHA to identify the stage of the disease, to diagnose comorbidities and opportunistic diseases, to provide primary prevention of opportunistic infections and to identify the necessity of ARV therapy. PLWHA with indications for ARV treatment underwent blood biochemical testing.

RESULTS: 74 PLWHA have been tested, 21 of which have CD4+ <200/µl , 6 have Hepatitis-B, 29 - Hepatitis-C, and 9 are co-infected with both infections. After clinical examination it is turned out that 7 patients have oral candidosis, 5 have herpes simplex, 4 have tuberculosis, 2 have coccidioidomycosis, 1 have cryptococcosis, 1 have MAC.

CONCLUSIONS: Analysis of the data of these 21 patients has shown, that average duration of the disease from the time of possible infection is 5.4 years, and it is noteworthy that 2/3 of them are injecting drug users. The study results allow identifying those PLWHA who are currently in need of ARV treatment and forecast those who will need ARV treatment in the near future.
