HIV-specific mucosal and cellular immunity in HIV-seronegative partners of HIV-seropositive individuals.

Abstract: HIV-specific mucosal and cellular immunity was analyzed in 15 heterosexual couples discordant for human immunodeficiency virus HIV) serostatus and in 50 randomly selected and presumably HIV- not exposed healthy controls. HIV-specific IgA were present in cervical swabs and/or urine of 14/15 (93%) HIV-exposed seronegative individuals; of 11/15 (73%) HIV-infected individuals; and of 4/50 (8%) controls. HIV-specific IgG were observed in cervical swabs and/or urine of 1/15 (7%) HIV-exposed seronegative individuals; of 15/15 (100%) HIV-infected patients; and of 4/50 (8%) controls. HIV env peptides-stimulated peripheral blood mononuclear cells (PBMC) of HIV-exposed seronegative individuals produced significantly more IL-2 (p.0009) and significantly less IL-10 (p .01) compared to HIV-infected individuals. Neither env peptides-stimulated IL-2 nor IL-10 production was detected in controls. These data demonstrate the concomitant presence of HIV-specific, IgA-mediated, mucosal-confined humoral immunity and HIV-specific systemic cell-mediated immunity in repeatedly-exposed HIV- individuals. These data thus confirm that a compartmentalized immune response to pathogens is possible in humans and raise the possibility of protective roles of CMI and mucosal IgA in HIV- individuals exposed to HIV.
