Incidence of Hodgkin's disease (HD) in a cohort of HIV seroconverters (Meeting abstract).

Abstract:
Incidence rates of HD in HIV-infected individuals with known date of seroconversion are an important contribution to evaluate whether an increase of HD due to the AIDS epidemic really exists. This study is part of a larger Italian multicentre investigation aimed at assessing the incidence of AIDS in a cohort of seroconverters for HIV infection. In the present analysis, the incidence of HD in such a cohort was evaluated. The number of observed HD cases was compared with that expected on the basis of average annual HD incidence rates (sex and age specific) registered in the general population of Italy. 1382 individuals (971 males and 411 females), 20 to 49 years of age, in whom the date of HIV seroconversion was estimated as the mid point between the dates of a seronegative and a seropositive serologic test for HIV antibody (median time 8 months, range 1-24 months), were followed for up to 14 years. For each individual enrolled in the study, the contribution in terms of person-years (PY) to sex and age specific groups was computed. The expected number of HD cases was drawn from three population-based cancer registries (representatives of the northern, central and southern areas of Italy) at the beginning of the AIDS epidemic (1985-1987). The ratio between observed (O) and expected (E) HD cases (O/E), and its 95% confidence interval (CI), was calculated. The 1382 HIV seroconverters contributed a total of 7375.39 PY of observation (5307.25 PY for males and 2068.14 PY for females). Three individuals developed HD during the follow-up period, for a yearly incidence of 4.07 HD cases per 10,000 PY. In comparison with rates in the general population, 0.082 cases of HD were expected, and the O/E ratio was 36.6 (95% CI: 0.0-78.12). In conclusion, although the power of the study was low, the higher frequency of HD among HIV seroconverters is of a magnitude similar to that reported at the beginning of the epidemic for some types of non-Hodgkin's lymphomas. This finding strongly suggests that HD, or some histologic subtypes, in HIV-infected individuals may be considered a further neoplastic complication of AIDS.
