Total pain's evaluation (TPE) in 40 AIDS patients with cancer (Meeting abstract).

Abstract:
Without a cure on the horizon there is a need to identify ways to sustain hope, existential and spiritual well being in pts with AIDS and cancer (Carson V, Perspect Psychiatr Care; 26(2):28-34 1990). Proposal of the method to evaluate the total pain was published in Proc Asco; 11:386 1992. Despite the importance of the sense of life, the transcendency, most of the existing scales do not assess spirituality. 'We suggest that the spiritual domain also needs to be tapped' (Donovan KJ, Clin Onc; 7(7):959-62 1989). In this study the TPE was used in 40 pts. 32/40 had Kaposi's sarcoma and 8/40 had non-Hodgkin's lymphoma (The major risk factors for HIV are homos cont in 29, IVDU in 8, heter cont in 3). 15% of pts have a maladjusted response in the spiritual aspects (without sense of life). So in 85% of pts may be sustained hope in spite of the human suffering of the AIDS and cancer. This technological and moral crisis that extends to the present medicine (as the AIDS crisis is demonstrated so well) sometimes, dehumanized, in which the spirituality of pts seems to be ignored as exemplified by Young D (J Nat Can Inst; 84(10):810 1992) as follows: 'There are physicians who disregard the ethical principles in order to maintain a very lucrative chemotherapy practice. I believe that the time has come to publish a code of ethical standards.'
