Abstract: Based upon the antiangiogenic properties of both interferon-alpha (IFN-a) and 13-cis-retinoic acid (CRA), together with the effectiveness of IFN-a against benign (capillary hemangiomas of infancy) and malignant (HIV-related Kaposi's sarcoma) vascular lesions, a combination of IFN-a and CRA was used to treat 16 patients (pts) with various tumors of vascular origin. Pts with the following diagnoses were treated: angiosarcoma (AS) of the head and neck (5 pts), breast (1 pt), and heart (1 pt), hemangiopericytoma (HP) (4 pts), epithelioid hemangioendothelioma (E HE) (1 pt), and benign hemangiomas (3 pts). The median age was 51 (range 12-80) and there were 10 males. All pts were good performance status and 3 pts had received prior chemotherapy or XRT. The dosages were IFN-a, 6 x 10(6) IU sc daily and CRA 1 mg/kg po in two divided doses. The clinical results are tabulated in a table. Eleven pts received treatment for 2 months or more. Four pts (AS-2 pts, HP-2 pts) were discontinued after one month because of progressive disease. Two pts with minimal disease AS of the face manifest brief clinical responses, and two pts with AS of the head had stable disease for 4 and 12 mo. A pt with hemangioma had relief of pain and radiologic improvement for greater than 12 mo. The drugs were tolerated poorly by elderly pts, the major side-effects being chronic fatigue and cheilitis. Myelosuppression was modest and uncomplicated. The clinical activity in AS of the head and neck is being pursued. Additional experience in pts with the more indolent HP, EHE and benign hemangiomas should be acquired.
