Abstract: There has been a recent demise in antiretroviral monotherapy and the ascendancy of combination therapy for HIV-infected patients. A few of the trials that established the change and a few that may contribute to the continuing evolution of antiretroviral therapy are briefly reviewed. The effect of antiretroviral therapy on the immune response and whether immunomodulators can be considered antiretroviral agents are explored. Finally, suggestions are provided on how medicine can move forward with the newly-developed agents and how clinical trial design might evolve in light of the extraordinary advances made in the past year.
