Abstract: The incidence of primary lymphomas of brain is increasing. The incidence of primary meningeal lymphoma is approx 1% of these. Review of literature indicates no report of follicular primary meningeal lymphomas. We report a follicular large cell lymphoma of the meninges presenting as a mass in a nonimmunocompromised young female. A 33-yr-old black female presented with a history of headaches of short duration. MRI of the brain revealed 2 dural based masses in left frontal area and lateral to right cavernous sinus. The patient underwent craniotomy with removal of the frontal mass. Histology revealed the tumor was arising from dura and was consistent with follicular large cell type. Immunoperoxidase studies showed positivity for CD20 (L-26), CD43 (Leu-22) and slight positive staining for kappa light chain. Lambda immunoglobulin light chain and CD3 were negative, consistent with B cell phenotype. The following studies were negative: bilateral iliac crest bone marrow biopsy, CT of chest and abdomen, spinal fluid cytology, right axillary lymph node biopsy and biopsy of a skin lesion. HIV studies were negative, and CD4 and CD8 counts were normal. Patient was treated with whole brain radiation and CHOP with complete disappearance of the second mass. Primary meningeal lymphomas are uncommon. We report a case of follicular large cell lymphoma of the meninges in a nonimmunocompromised young female.
