Abstract: John S. James proposes a program for the careful and consistent monitoring of experimental AIDS treatments already in use by patients and physicians. The proposal's goal is to greatly reduce the time and cost of accessing the first widely credible information about the effects of new treatments, combinations, and treatment strategies. The paper outlines the protocol, presents ethical considerations, and responds to specific questions which may arise from this type of screening study, such as viral load effects from using acyclovir; the value of certain micronutrients on disease progression; and the benefit of doxycycline, hydroxyurea, and ribavirin in AIDS treatment. It is suggested that a working monitoring project will allow quicker, more consistent results without the problems of selection bias.
