Abstract: 106 patients affected by the classic type of Kaposi's sarcoma all born and living in North-East Sardinia have been studied. In 70 patients the disease was circumscribed and in 36 it was disseminated. Reduced number of CD4+ and increased number of CD8+ T-lymphocytes was observed; CD4:CD8 ratio was 1.40 +/- 0.5 in the patients with circumscribed disease and 0.77 +/- 0.2 in the patients with disseminated disease (1.78 +/- 0.6 among the controls). Tuberculin reactivity was decreased in patients with the disseminated disease. 72 class I and class II HLA antigens have been typed. No significant variation of class I antigens was observed; among class II antigens DR-3, DR-5, DQw2 and DQw3 recurred with altered frequency. DR-3 frequency was 18.8% among all the patients against 53.6% among the controls; DR-5 was 56.6% against 23.1%; DQw2 21.7% against 60%; DQw3 76.4% against 54.1%. The analysis of split DR-5 demonstrated that DR-11 was present in 41.9% of the patients against 77.8% of the controls and DR-12 was present in 58.1% of the patients against 22.2% of the controls. The concurrent decrease of CD4:CD8 ratio, of tuberculin reactivity and of HLA DR-3 antigen frequency in patients with the disseminated type of the disease could be interesting, also remembering that DR-3 is frequently observed in patients affected by autoimmune diseases.
