Abstract: In recent years, techniques, probes and reagents became available to reliably visualize EBV-infected cells, to assess EBV gene expression, and to characterize the clonal composition of EBV genomes in human tissues. Application of these techniques to lymphoproliferative lesions revealed (1) an association of EBV with various mono- and oligoclonal lymphoproliferations ranging from benign to overtly malignant lymphomas, and (2) three characteristic patterns of EBV gene expression among EBV-associated lymphoproliferations, ranging from a minimal to the full spectrum of EBV latent gene products. In view of the established immortalizing and transforming properties of EBV, the findings support the concept of an etiologic role of EBV for certain B- or T-cell lymphomas such as Hodgkin's disease, anaplastic large cell lymphoma, and lymphomas arising in immunocompromised individuals. Because proportions of the EBV infected cells in many of these cases express the transforming protein LMP-1 (latent membrane protein), the virus may influence the proliferative properties as well as the morphology and phenotype of the neoplastic cells. The expression of viral neo-antigens known to represent targets for cytotoxic T-cells reflects either specific defects in the immune response or global immunodeficiencies as seen in HIV-associated disease.
