Abstract: Thymopentin (TP5), a synthetic pentapeptide corresponding to the active site of the human thymic hormone, thymopoietin, has demonstrated immunomodulatory activity in autoimmune diseases, immune deficiencies, and HIV infection. A recent study reported a 75% response rate to TP5 in patients with the Sezary syndrome who had low numbers (less than 2600/mm3) of circulating Sezary cells. We further explored the clinical and biological activity of TP5 in heavily pretreated patients with advanced cutaneous T-cell lymphoma (CTCL) with and without the Sezary syndrome. All patients received 50 mg of TP5 by intravenous infusion three times a week until progression. Of 16 treated patients, 15 had failed prior chemotherapy; 8 had Sezary syndrome, 2 had erythroderma without Sezary cells, and 6 had plaques or tumors. One patient with ulcerated plaques had a partial response with healing of skin lesions, and 7 patients experienced minor response (6 in skin, 1 in nodes) or symptomatic improvement of pruritus. We conclude that TP5 has bioactivity in CTCL and we are currently studying the effects of TP5 on antigen expression on Sezary cells and its immunomodulatory effects on normal lymphoid subsets.
