BOMES: a promising combination chemotherapy in the treatment of CNS lymphoma (Meeting abstract).

Abstract: Although with occasionally excellent results with corticosteroid treatment, the response of CNS lymphoma to systemic chemotherapy remains unsatisfactory. This is presumably partly due to the blood-brain barrier (BBB). To cope with this, we designed a protocol which, in addition to high-dose corticosteroids, consisted of drugs that cross BBB. The regimen included BCNU 65 mg/m2 iv D1,2; vincristine 2 mg iv D1,8; etoposide 50 mg/m2 iv D1-5; methylprednisolone 200 mg/day iv D1-5; and methotrexate 1.5 gm/m2 iv D15, followed by leucovorin rescue. The course was repeated every 3 wk. All patients were anti-HIV(-), and 5 patients had systemic lymphoma involvement. The histology type included 8 diffuse large cell, 1 small noncleaved, and 1 primary meningeal lymphomas. Chemotherapy and cranial irradiation has been previously given in 3 and 4 patients, respectively. Myelosuppression was the limiting toxicity, with ECOG Gr3-4 leukopenia developed in 50% of 32 courses given and Gr3 infection in 2 patients. One hepatitis B carrier died of fulminant hepatitis. CR and PR were seen in 6 and 3 patients, respectively; with a response rate of 90% (56%-99%, 95% CI). The remission duration of complete responders was 1+, 2, 3+, 7, 20+, 30+ mo, respectively. We suggest that BOMES is an effective regimen in the treatment of CNS lymphoma. Further study in a larger group of patients is indicated.
