Etiology of acute abdomen in AIDS patients.

Abstract: OBJECTIVE: To report the clinical characteristics and the etiology of acute abdomen in a group of AIDS patients, who had been submitted to exploratory laparotomy. METHODS: Retrospective study compresing the period from march 1st, 1988 to june 30, 1993, at the Hospital de Infectologia, IMSS, in Mexico City. We reviewed the clinical records of 29 patients with diagnosis of AIDS and exploratory laparotomy. We analyzed demographic, clinical and surgical variables. RESULTS: Twenty eight were men and one woman. Abdominal pain was present in all patients. Lymphopenia ocurred in 81% of the cases. The etiology of the acute abdomen was divided in: 1) common to the general population, such as, acute appendicitis, 2) secundary to opportunistic infections, like intestinal perforation due to Cytomegalovirus and 3) secondary to neoplasia, such as, non Hodgkin ulcerated and perforated lymphoma. DISCUSSION AND CONCLUSIONS: The postoperative mortality in this cases is associated with the etiology agent of the acute abdomen, age of the patients and advancing of the illness. It necessary to evaluate very carefully the AIDS patients to diagnose at the earliest surgical complications.
