MGBG: a highly active drug in relapsed or refractory AIDS-lymphoma (Meeting abstract).

Abstract:
Methyl-glyoxal-bisguanylhydrazone (MGBG) interferes with polyamine biosynthesis, thought important in the stabilization of DNA. Used initially in the early 1980s on a daily or weekly basis, response rates of 29-40% were seen in 'de novo' lymphoma, although toxicity (mucositis, vasculitis) was significant. With a long T1/2, good penetration into CSF, and relative lack of marrow toxicity, a dosing schedule of MGBG, 600 mg/m2 IV days 1, 8 and then every 2 wk was used in 10 male pts with refractory or relapsed AIDS-lymphoma. Median age was 41 yr (26-58). Median CD4 cells prior to MGBG was 117/dl (4-427); 80% had CD4s less than 200/dl. Four pts had AIDS prior to MGBG. Four pts had 1 prior regimen for AIDS-lymphoma, 4 had 2; 1 had 3; and 1 had 4 regimens prior to MGBG. High grade small noncleaved lymphoma was present in 6; 1 had B immunoblastic; others had intermediate grade. Stage IV was present in 9 (BM in 3; CNS in 2; lung in 5+ others). A median of 4+ doses of MGBG was administered (1-20 +). Toxicity was mild, consisting of flushing (N=9); tingling (N=8); euphoria (N=3); or somnolence (N=2) during infusion. Mild nausea, vomiting (N=4); fatigue (N=3); and neutropenia (N=3) were also seen. Response: CR was seen in 2, solid PR in 3, for response rate of 50% in these heavily pretreated pts. Quality of survival is excellent, and median duration survival since MGBG is 4.3 mo. We conclude: (1) MGBG is associated with 50% durable response rate in refractory AIDS-lymphoma; (2) Toxicity is mild; (3) Quality and quantity of survival have been positively impacted; (4) Study is on-going with additional pt entry.
